This privacy notice explains what information we will collect about you through participation in the community survey for the Pharmaceutical Needs Assessment (PNA), how we store this information, who we share it with and how we keep it safe and confidential. We want you to be confident that your information is kept safe, secure and for you to understand how and why we use it. It sets out how Knowsley Council will use data collected from respondents.
You can decide whether to complete the survey or participate. You can decide how much or how little you wish to participate. Please be aware that, because we do not ask for information that would directly identify you, it is unlikely you would be able to withdraw your answers after you have returned the survey.
Please note that there are questions within the survey that use free text boxes. Please do not put any personal identifiable pieces of information in here such as your name, address, or other identifiable aspects about yourself.
For question 2 please do not input your full postcode and just use the first prefix for example L36.
Completing the survey is a voluntary act on your part to help us comply with the Knowsley Health and Wellbeing Board’s (HWB) statutory responsibility to produce a new PNA every three years.
The draft PNA must undergo a 60-day statutory consultation and this survey ensures that we comply with this obligation.
Who will own my data once I submit it?
Liverpool City Council is the data controller for the information that you provide. They are running the survey and will securely share the information that relates to Knowsley residents with Knowsley Council for our own analysis. At this point Knowsley Council will become the independent data controllers. All analysis of the data that relates to Knowsley Council will be undertaken by Knowsley Council’s Intelligence and Insight Team.

Why do you need my information?
The Pharmaceutical Needs Assessment is a statutory document that is produced under the guidance of the Health and Wellbeing Board. It is used to inform NHS England and local providers about the sufficiency of pharmacy provision in a local area. The information gathered in the survey adds to the understanding of the sufficiency of pharmacy provision across small local areas to allow for producing assessments of the health and care needs of the population, in particular to support the statutory responsibilities of the:
- Pharmaceutical Needs Assessment
- Joint Strategic Needs Assessment (JSNA)
- Director of Public Health’s Annual report
- Identifying priorities for action
- Informing decisions on (for example) the design and commissioning of services
- To assess the performance of the local health and care system and to evaluate and develop them
- To report summary statistics to national organisations (NHS England) and the Cheshire and Merseyside ICB. This will be headline, aggregated data and identifiable data will not be shared.
- Undertaking equity analysis of trends, particularly for vulnerable groups.
In these cases, the information is used in such a way that individuals cannot be identified from them and personal identifiable details are removed as soon as is possible in the processing of intelligence on an aggregated level.
What type of information will you be processing?
When completing the survey, we may process parts of your personal data. The survey may appear to be anonymous as we will NOT be asking for any specific personal details that would directly identify you, for example, your name, date of birth, full address. However, we may be able to identify you through the collective answers you provide indirectly on the survey, and we are required to make you aware of this.
We ask optional questions for equalities monitoring, including:
- Age group
- Gender
- Carer Status
- Disability
We also ask identifiable questions that include:
- Postcode
- Disability including Cancer, Diabetes or HIV
- Health Condition including impairments
- Ethnic Group
- Religion
- Sexual Orientation
In almost all instances you have a “Prefer not to say option”
Your participation is entirely optional, by participating you are consenting to the information provided being used for the purposes stated.
What allows you to use my information?
- Completing the survey is a voluntary act on your part to help us understand sufficiency of pharmacy provision.
- By completing the survey, you are giving your consent for the data you give to be used.
The legal bases which allows the Council to lawfully process your data fall under the UK General Data Protection Regulation.
Article 6(1) (a) Consent “the individual has given clear consent for you to process their personal data for a specific purpose”.
By choosing to complete the survey, you give Knowsley Council consent to process the data you provide. As previously mentioned, this data will in the main be anonymous and due to this reason, it is unlikely you would be able to withdraw your consent once entered.
Article 6(1) (e) Public task “processing is necessary for the performance of a task carried out in the public interest or in the exercise of official functions, and the task or function has a clear basis in law.” As outlined in the Levelling-up and Regeneration Act 2023.
Who will my information be shared with?
Aggregated and anonymous data gathered from this survey will be used by Knowsley Council in the final Pharmaceutical Needs Assessment, which will be published online. The final document will not contain any personal or identifiable data at any level.
Do I have to provide this information and what will happen if I don’t?
No, participation is entirely voluntary and not a legal requirement. You can decide whether to participate or not, and you can decide how much or how little you wish to participate. There is a ‘prefer not to say’ option available.
How long will you keep this data for and why?
The Survey will be active for one month Liverpool Council will maintain the survey until 31st December to accommodate all Local Authorities in Cheshire and Merseyside. Data will be securely transferred to Knowsley Council after this for analysis.
Data will be held by Knowsley Council for a period of up to 9 months after publication of the Pharmaceutical Needs Assessment following the publication of the final document on 1st October 2025. Any paper copies will be destroyed through confidential waste, and electronic records will be securely deleted also.
How will my information be stored?
Survey data held by Knowsley Council will be stored on a secure Council owned electronic system. Paper (hard) copies will be scanned and uploaded to the secure system and copies will be destroyed through confidential waste.
Will this information be used to take automated decisions about me?
No.
Will my data be transferred abroad and why?
No.
What rights do I have when it comes to my data?
Under the Data Protection Act (DPA) 2018/ UK General Data Protection Regulation (UK GDPR), you have the following rights with regard to your personal data: -
- The right to subject access – you have the right to see a copy of the personal data that the council holds about you and find out what it is used for
- The right to rectification – you have the right to ask the council to correct or remove any inaccurate data that we hold about you.
- The right to erasure (right to be forgotten): you have the right to ask the council to remove data that we hold about you.
- The right to restriction – you have the right to ask for your information to be restricted (locked down) on council systems.
- The right to data portability – you have the right to ask for your data to be transferred back to you or to a new provider at your request.
- The right to object – you have the right to ask the council to stop using your personal data or to stop sending you marketing information or complain about how your data is used.
- The right to prevent automated decision making – you have the right to ask the council to stop using your data to make automated decisions about you or to stop profiling your behaviour (where applicable)
