Why Do You Need My Information?
- To demonstrate that we have consent to provide the service to identified children
- To be able to contact parents / carers
- To have a better understanding to the need of the identified child
- To provide support to educational settings to address the needs of the identified child
What Allows You To Use My Information?
Parents/ carers sign a referral form that gives consent for the involvement of the Hospital and Home Tuition Service and to share the information with other professionals involved with their child.
Who Will My Information Be Shared With?
- Educational staff – SENCo, Head teacher, class teacher, Educational Psychologist
- Health staff (if appropriate)– Speech and Language Therapist, Community Paediatrician, Occupational Therapist
Do I Have To Provide This Information And What Will Happen If I Don’t?
Parents and carers have a choice if they want to access the Hospital and Home Tuition Service for their child. If they do not wish to give consent for this then the service will not offer support. Parents and carers have the right to withdraw consent for this service at any time.
How Long Will You Keep This Data For And Why?
The information will be kept until the child reaches the age of 25 in order to comply with the Local Authority archiving procedures.
How Will My Information Be Stored?
Initially information will be stored in files in a locked filing cabinet and once the service is no longer required for the child, the information will be scanned and stored on the Local Authority Sharepoint system.

What Rights Do I Have When It Comes To My Data?
Under the Data Protection Act (DPA) 2018/ UK General Data Protection Regulation (UK GDPR), you have the following rights with regards to your personal data:

- The right to rectification – you have the right to ask the council to correct or remove any inaccurate data that we hold about you.
- The right to erasure (right to be forgotten) you have the right to ask the council to remove data that we hold about you
- The right to restriction – you have the right to ask for your information to be restricted (locked down) on council systems
- The right to data portability – you have the right to ask for your data to be transferred back to you or to a new provider at your request
- The right to object – you have the right to ask the council to stop using your personal data or to stop sending you marketing information, or complain about how your data is used
- The right to prevent automated decision making – you have the right to ask the council to stop using your data to make automated decisions about you or to stop profiling your behaviour (where applicable)
