Notify a medical condition
If you are a private hire driver and need to inform us of a new medical condition since your badge was issued, please complete the Change of Medical Details form below.
When completing this form, you must:
- Upload valid medical evidence from a General Practitioner with access to your medical records
Please see the DVLA Medical guidance for Group II licence rules. Once you have submitted your change of medical details form, you will be contacted by email within ten working days. If you need to provide further evidence in support of your request, please submit it using the Upload Evidence Facility.
