The Short Breaks Individual Payment is part of a pilot scheme that Knowsley Council are running during 2021/2022 alongside the commissioned Short Breaks groups, providing additional support for families. It will allow families to make choices about the type of activities/support that works best for them. It is not associated with, nor will it affect any assessed Health or Social Care direct payment or personal budget you are in receipt of.

Why Do You Need My Information?
As part of the Council’s Short Breaks Individual Payment pilot to support access to respite opportunities for children with disabilities and their families, the following information is required. This will ensure that the one-off Short Breaks Individual Payment can be made to the correct individual, the individual is contactable as part of the monitoring requirements and it will help evidence the impact, performance, and success of this pilot:
- Name of parent/carer and their child/ren
- Full residential address
- Email address
- Mobile telephone number
- Home telephone number
- Bank details including Account name; Account number; Sort code
In addition, under the terms of the agreement to process the payment, we require completion of a monitoring form to enable us to monitor the effectiveness of the pilot. As part of this, we would like to use your feedback of how the Short Breaks Individual Payment has positively impacted you. For example, feedback may be shared with Local Authority partners such as Adult Social Care or CCG to demonstrate its success and apply for future funding to continue the pilot. However, this element of the form is subject to your consent and you do have to option to opt-out which is clearly marked on the form.
What Allows You To Use My Information?
All organisations must provide a legal basis for processing your information. In most cases this is found under the Data Protection Act 2018, including the UK General Data Protection Regulation (UK GDPR).
You will be required to complete and return the Short Breaks Individual Payment Agreement to allow the Council to process your data for you to receive the one-off Short Breaks Individual Payment. As a requirement of the agreement, completion of a monitoring form is required which includes mandatory information to be provided on request within 14 days to monitor the performance of the pilot. As such, the processing is necessary for the performance of a contract to which the data subject is party.
The request to use/share your feedback as part of the monitoring form is subject to your consent and you are able to opt out or withdraw your consent to this element of the processing using the contact details provided on the form.
Who Will My Information Be Shared With?
Your information may be shared with the following partners:
- Services within the Local Authority e.g., Whole Life Commissioning Team, Business Support and Finance - in order to process the one-off Short Breaks Individual Payment and for audit monitoring purposes.
- Children’s Social Care e.g., Short Breaks Support Officer and Children with Disability Team – in order to record individuals who are in receipt of the Short Breaks Individual Payment.
- Adult Social Care and CCG – For example, feedback will be used to demonstrate the positive impact of the Short Breaks Individual Payment pilot in support of future funding for the project.
All personal identifiable information in relation to your feedback, will be anonymised prior to being shared.
Do I Have To Provide This Information And What Will Happen If I Don’t?
By completing and returning the necessary forms, it enables you to receive the one-off Short Breaks Individual Payment. Failure to do so, will mean that Knowsley MBC will be unable to process the Short Breaks Individual Payment. Within the monitoring form, you are under no obligation to share feedback of how the Short Breaks Individual Payment has been utilised, however this information would support the request for future funding from partners.
How Long Will You Keep This Data For And Why?
Your information will be stored for up to 6 years from completion of payment to achieve the relevant purpose, after which it will be securely destroyed. The majority of this information is kept electronically.
How Will My Information Be Stored?
Your information will be stored securely on our IT system. Some information is held in a central SharePoint site. Access to the systems is restricted to those who are entitled to view and process your data. If the information is collected in a paper format, we will also ensure it is kept secure and destroyed when no longer required.

What Rights Do I Have When It Comes To My Data?
Under the Data Protection Act (DPA) 2018/ UK General Data Protection Regulation (UK GDPR), you have the following rights with regards to your personal data:

- The right to data portability – you have the right to ask for your data to be transferred back to you or to a new provider at your request

- The right to prevent automated decision making – you have the right to ask the council to stop using your data to make automated decisions about you or to stop profiling your behaviour (where applicable)
